Mommy makeover in Colombia: one operation, not a package

Abdomen, breasts and contour in a single surgical plan. The question is not how many procedures fit in the brochure, but how many fit safely into one operation — and when two stages are the better answer. By Dr. Daniel Ramos, SCCP board-certified plastic surgeon.

The Procedure

What a mommy makeover is: three fronts, one plan

Pregnancy does not leave one aesthetic problem behind: it leaves several, and they are almost never solved by the same technique. It leaves separated rectus muscles — the diastasis, which no amount of core work closes, because the problem is not strength but the distance between the muscle bellies; it leaves lower abdominal skin that no longer retracts; and it leaves breasts with less volume and more descent than before. A mommy makeover is the plan that puts those fronts in order: what gets corrected, with which technique and — the part few brochures mention — how much fits safely into one operation. Because combining is not free, and the decision to do it all at once is made with numbers, not enthusiasm.

01Abdomen: plication of the diastasis and removal of excess skin
02Breasts: augmentation, lift or both, depending on volume and descent
03Contour: flanks, back and epigastrium to bring the waistline back

The Difference

A fixed package or a surgical plan

Both go by the name mommy makeover. One is built so the figure looks good in a post. The other is built from your abdomen, your breasts and your hours of theatre time.

The brochure package
  • The price is published before anyone has examined the diastasis or measured the excess skin.
  • Procedures are added because they add to the sale, not because the case needs them.
  • Nobody mentions total operative time, or what that time does to the risk.
  • "One single recovery" is promised, leaving out that it is two recoveries happening at once.
The surgical plan
  • Each front is examined separately and the technique it needs is defined: plication, lift, implant, liposuction.
  • Venous thromboembolism risk is scored with a validated tool and prophylaxis is defined before surgery.
  • Total operative time is projected and what fits safely into one stage is decided from it.
  • When the full plan demands too many hours, staging is proposed — even though it is the less marketable option.

Each component has its own page, with its technique, recovery and risks explained in full: tummy tuck, breast augmentation and liposuction. The mommy makeover is the decision about how they are combined.

The most frequent combinations

01 · Most common

Abdomen + breasts

Tummy tuck with rectus plication and, depending on the case, breast augmentation or a lift. It is the classic combination and, in low-risk patients with a reasonable operative time, it is done in one stage.

02 · When the breasts held up

Abdomen + contour

When breast volume and position were preserved, the plan concentrates on the abdomen and the waist: plication, skin, and liposuction of the flanks, back and epigastrium.

03 · When staging is right

Two surgical stages

When the full plan exceeds reasonable theatre hours, or when individual risk is high, it is split into two operations months apart. It is the option that gets promoted least and is sometimes the correct one.

What the evidence says about combining

This is the part that does not appear in package posts, and it is exactly the part worth reading before you decide. Combining procedures has real benefits — one anaesthetic, one recovery — and a measurable cost in risk.

Combining · the actual number

In a series of nearly 25,000 abdominoplasties, the major complication rate was 3.1% for the tummy tuck alone and 10.4% when combined with body contouring plus liposuction. After adjustment, combining procedures carried a 50% higher relative risk of major complications. It is the reason total operative time gets projected before anything is promised.

Thrombosis · why it is scored

Venous thromboembolism risk in plastic surgery is stratified with the Caprini score. In the validation series, patients scoring above 8 had an 11.3% rate of symptomatic VTE at 60 days without chemoprophylaxis — and events appeared throughout that period, not only in the first few days. Hence scoring every patient, defining prophylaxis and insisting on walking from day one.

Pannucci CJ, Bailey SH, Dreszer G, et al. J Am Coll Surg. 2010;212(1):105-112
Diastasis · why the gym does not fix it

What pregnancy separates is the linea alba, the tissue joining the two bellies of the rectus abdominis. The muscle can be strong and the separation still be there: that is why a trained abdomen can still bulge when it contracts. Closing it requires plicating the fascia with sutures, and that repair is what cannot take weight during the first weeks.

Surgical criterion · abdominal wall assessment

None of this is meant to frighten you. It is so that when you are shown two quotes with different figures, you know what to ask: how many hours of theatre time, what prophylaxis, and why this plan rather than another.

In a mommy makeover that matters for an unglamorous reason: a surgeon's value is not in how many procedures he agrees to stack, but in knowing when to say no and offer you two stages instead of one. What we do not do in a mommy makeover

  • We do not publish fixed-price packages. A figure set before anyone has examined your abdomen and your breasts is not a quote: it is advertising. The quote comes out of the consultation and states exactly what it includes.
  • We do not stack procedures to make the figure look better. Every added procedure adds theatre hours and adds risk. If your full plan does not fit safely into one stage, we propose two — and explain why.
  • We do not operate while weight is still moving or breastfeeding is ongoing. Stable weight for at least six months and breastfeeding finished. Operating earlier means building a result that is out of date before the scars have matured.
  • We do not give a date until support at home is sorted. No lifting — children included — for four to six weeks. If that logistics problem is unsolved, the muscle repair is what pays for it.

What does a mommy makeover cost in Colombia?

There is no price for a mommy makeover, because there is no such thing as the mommy makeover: there is yours, with the components your case needs. What we can tell you precisely is what it depends on.

01Which components it includes. Abdomen with plication, breasts with an implant or a lift — or both — and how many liposuction areas. Each front changes the figure and the time.
02One stage or two. Staging costs more in total, but it is the right call when the full plan exceeds reasonable theatre hours or when your risk demands it.
03Implants, where used. Brand, profile and volume are chosen at the consultation and included in the quote — they never turn up afterwards as an extra.
04What the quote includes. Surgeon's fee, accredited surgical facility, anaesthesiology, implants where used, supplies, compression garment, anti-embolism stockings, prophylaxis and follow-up through the first year.

The consultation — in person or virtual — is the only point from which a responsible figure can be given, because the plan depends on what your pregnancy left behind and on how much fits safely into one operation.

To be precise: these are individual cases of each procedure, not of a combined operation. We present them this way because a mommy makeover before-and-after should show the same patient operated on both fronts in the same surgery, and we would rather not present separate cases as though they were one. Every patient is different: the result depends on the diastasis, the excess skin, the degree of breast descent and the recovery.

Frequently Asked

Everything about the mommy makeover

There is no fixed list, and you should be wary of anyone who gives you one before examining you. "Mommy makeover" is the commercial name for surgery that combines whatever pregnancy left behind in your case, and it usually covers three fronts: the abdomen — a tummy tuck with plication of the separated rectus muscles and removal of excess skin; the breasts — augmentation, a lift, or both, depending on how much volume was lost and how far the areola descended; and the contour, with liposuction of the flanks, back and epigastrium to bring the waistline back. What goes in and what stays out is decided at the consultation, not in a brochure.

It depends on which components your plan includes, on total operative time and on whether it is done in one stage or two. An abdomen with plication plus a breast lift does not cost the same as an abdomen plus an implant augmentation, and adding liposuction of several areas changes both the time and the figure again. What is comparable is what the quote includes: surgeon's fee, accredited surgical facility, anaesthesiology, implants where used, supplies, compression garment, anti-embolism stockings, prophylaxis and follow-up through the first year. Dr. Ramos gives you a personalised all-inclusive quote after the consultation. A package with a fixed price published before anyone has seen your abdomen is a commercial offer, not a surgical plan.

Sometimes yes and sometimes no, and the difference is total operative time and your individual risk. The data on what combining costs are clear: in a series of nearly 25,000 abdominoplasties, the major complication rate was 3.1% when the tummy tuck was done alone and 10.4% when it was combined with body contouring plus liposuction. That does not mean combining is forbidden — it means the decision has to be made on judgement, not to save an anaesthetic. That is why the consultation includes calculating your venous thromboembolism risk with a validated score, defining prophylaxis and, when the full plan demands too many hours of theatre time, proposing two stages instead. It is the least commercial conversation in the consultation and the most important one.

Two conditions, and both are firm. The first is breastfeeding: it must be finished, because the breast is still changing volume and any decision about an implant or a lift made on that tissue is out of date within months. The second is weight: stable for at least six months. Operating in the middle of significant weight loss means building an abdomen that will have excess skin afterwards. This is not a rule designed to make you wait: the result of this surgery is judged at twelve months, and over that period the body has to hold still.

In practice, this is the question that shapes the planning most. After a tummy tuck with rectus plication there is no lifting for four to six weeks, and a small child weighs a good deal more than the recommended limit. It has to be organised before surgery: who does the school run, who lifts the baby out of the cot, who stays the first few nights. We raise it at the first consultation rather than the week before, because the muscle repair depends on it. Patients who run into trouble at this point almost never do so because of the surgery: they do so because there was no help at home.

The abdominal one is the most visible and the most honest to explain: a low horizontal line, designed to hide under underwear, which usually incorporates the caesarean scar where there is one, plus a small scar around the umbilicus. At the breast it depends on the component: an implant augmentation leaves a short scar in the inframammary fold; a lift adds a scar around the areola and, depending on the degree of descent, a vertical one down to the fold. Liposuction leaves incisions a few millimetres long. All of them are drawn with you before surgery. Scars mature between six and twelve months, and what is done during that time — sun protection, massage, silicone — does change how they end up.

Technically a later pregnancy is possible and is not dangerous because you were operated on, but it undoes much of the work: the rectus plication stretches again, the abdominal skin stretches again and the breasts change volume again. If you have concrete plans for another pregnancy in the short or medium term, the honest answer is that you are better off waiting. If the question is about ten years from now, that is a different conversation. What we do not do is operate without asking, because it is one of the few variables that can turn a good result into a second surgery.

It is the longest recovery among the common aesthetic operations, and it is worth planning with room to spare. The first forty-eight hours involve early mobilisation — walking on the day of surgery is part of thrombosis prevention, not an optional suggestion — and a check-up at forty-eight hours. The compression garment is worn for six to eight weeks. Most patients return to desk work between the second and third week, with no lifting; impact exercise waits eight weeks. You will walk slightly bent forward for the first few days because of the plication, and that eases. The definitive contour and scar maturation are seen between six and twelve months. The detailed calendar for the liposuction component is in lipo 360 recovery, week by week.
