Rib remodeling in Colombia: the waist is also bone

When the waist will not change with diet, exercise or liposuction, sometimes what limits it is the floating ribs. Rib remodeling changes their angle without removing them. It is real surgery with real risks: here is what it does, who it is for and who it is not for. By Dr. Daniel Ramos, SCCP board-certified plastic surgeon.

The Procedure

What is rib remodeling? Moving the rib, not removing it

The waist has three layers: fat, treated with liposuction; muscle in the abdominal wall; and the skeleton, which in the lower chest means the floating ribs — the 11th and 12th, sometimes the 10th — which are not attached to the breastbone and flare outward. For years the only way to act on that last layer was to remove those ribs, an aggressive operation. Rib remodeling does something different: it creates a controlled, incomplete «greenstick» fracture that lets the rib tilt inward without being removed. While the bone heals, a compression garment or corset holds the new position. Depending on the technique it is done through punctures a few millimetres wide or through a small incision in the back — and almost always together with liposuction, because the rib is only one part of the waist.

01It works on the skeleton, not on fat: it is rarely done alone
02The rib is repositioned, not removed
03It carries chest-specific risks, and they are weighed before, not after

The Difference

Removing the rib or changing its angle

Both are sold as ways to «narrow the waist». They are not the same operation, they do not carry the same risk, and they do not leave the same result.

Rib removal
  • The floating ribs are taken out through a larger incision: bone that is removed does not come back.
  • It is the oldest and most aggressive way to act on the waist through the skeleton.
  • It has described risks of hemothorax and pneumothorax because of how close the pleura is.
  • It leaves the side wall of the trunk without the support those ribs provided.
Rib remodeling
  • The rib is kept and its angle is changed with a controlled partial fracture.
  • The approach is through punctures or a small incision, depending on the technique.
  • The garment or corset holds the new position while it heals: it is part of the treatment.
  • It is planned together with liposuction, because fat and rib are treated differently.

Before any of this comes the question that decides the operation: what is limiting your waist? If most of it is fat, the answer is high-definition lipo 360, not this surgery. If the abdomen bulges because the rectus muscles are separated or there is excess skin, what you need is a tummy tuck.

What limits the waist in each case

01 · Most common

Fat

Fat on the flanks, waist and lower back. Treated with liposuction and, when definition is the goal, with high-definition technique. Many waists change enough with this alone.

02 · Where this surgery comes in

Ribs

A wide rib cage or flared floating ribs that set the limit of the waist even with little fat. This is the indication for remodeling, almost always combined with liposuction.

03 · What has to be said

Muscle and skin

Diastasis recti, excess skin, or a belly that sticks out because of internal fat. Neither the rib nor the cannula fixes that: it is a different operation, or not an operation at all.

What the literature says so far

Rib remodeling is a young operation: the first published description dates from 2021, and almost all the evidence consists of case series by the surgeons who developed it. That does not invalidate it, but it means it has to be described carefully.

The origin · 2021

A Moscow group described narrowing the waist through a controlled fracture of the 11th and 12th ribs without removing them, as an alternative to resection, which they considered too aggressive for a contouring operation. The puncture, piezotome and ultrasound-guided techniques were later built on that idea.

Results · what gets measured

In the puncture-technique series from Bogotá, Bucaramanga and São Paulo, with 125 patients, the median waist went from 76 cm to 65 cm at three months. But almost all of them also had liposuction and there was no comparison group: the whole difference cannot be credited to the ribs.

Risks · what cannot be left out

A 2025 systematic review reported pneumothorax in 1.9%, skin burns in 6.7%, asymmetry in 2.3% and severe pain in 9.2%. In 2026, six deaths associated with the puncture technique in three countries were published, reviewed by committees that included the SCCP.

This is not meant to scare you: it is so you know why the consultation asks so many questions, why an accredited surgical facility is non-negotiable, and why sometimes the right answer to «I want a smaller waist» is a different operation.

The waist as a complete contour

Dr. Daniel Ramos Hernández is board-certified by the Colombian Society of Plastic Surgery (SCCP) and trained in Plastic, Reconstructive and Aesthetic Surgery at Universidad El Bosque. He completed the Total Definer fellowship with Dr. Alfredo Hoyos — the school behind high-definition liposuction, and one of the groups that has published on rib remodeling in Colombia. That matters here for a concrete reason: the rib is only one of the three layers of the waist, and the result depends on planning fat, bone and the transition to the hips and back together. What we do not do in rib remodeling

  • We do not recommend it when the waist is fat. If fat is what limits your waist, operating on the ribs exposes you to a risk that will not give you the result you want. We start by measuring how much is fat, how much is bone and how much is muscle.
  • We do not operate on patients the published series exclude. Chronic respiratory disease, previous rib surgery, a high body mass index, spinal problems or bone that does not heal well: those are the conditions the published evidence leaves out, and so do we.
  • We do not do it outside an accredited surgical facility. The serious complications of this operation are chest complications — pneumothorax, hemothorax — and they are handled with an anaesthesiologist, monitoring and equipment, not in an office.
  • We do not promise inches. Published series give ranges, not guarantees, and nearly always include liposuction. We tell you what is reasonable to expect in your case — including when the answer is that the rib is not what limits you.

Rib remodeling recovery and what to expect

What the literature describes — your personalised surgical plan sets the exact details for your case, especially if it is combined with another operation.

ApproachPuncture or small incisionDepending on the technique. Accredited surgical facility and board-certified anaesthesiologist.
Garment or corsetSeveral weeksContinuous. It holds the ribs in their new position while they heal.
PainLike a bad bruiseWhen coughing, laughing or turning, mostly in the first weeks. Managed with prescribed pain relief.
Measurable resultFrom month 3That is when published series assess it, once the bone has healed.
01Shortness of breath, worsening chest pain or difficulty breathing: seek care immediately, do not wait for the follow-up
02Neglecting the corset is one of the causes of asymmetry: it is not an accessory

How much does rib remodeling cost in Colombia?

There is no single price, because different techniques share the same name and it is almost never done alone. What we can tell you precisely is what it depends on.

01The technique. Puncture, small incision or fixation: the instruments, operating time and recovery all change.
02How many ribs. Treating the 11th and 12th is not the same as including the 10th, and not every waist needs it.
03What it is combined with. Adding high-definition lipo 360, male lipo 360 or a BBL changes the plan — and saves a second anaesthetic.
04What the quote includes. Surgeon fees, accredited surgical facility, anaesthesiology, supplies, garment or corset and follow-up visits — everything, with no costs appearing later.

The consultation — in person or virtual — is the only point from which a responsible figure can be given, because first we need to know whether your ribs are what limit your waist.

Frequently Asked

All about rib remodeling

It is a body-contouring operation that narrows the waist by working on the skeleton rather than on fat. A controlled, incomplete «greenstick» fracture is made in the floating ribs — usually the 11th and 12th, sometimes the 10th — so that they tilt inward and the waist becomes narrower. Current techniques do not remove bone: they reposition it, and a compression garment or corset holds the new position while it heals. That is why it is usually presented as an alternative to rib removal, which is more aggressive.

It depends on your anatomy and on what is done at the same time. In the published series of the puncture technique from Colombia and Brazil, with 125 patients, the median waist went from 76 cm to 65 cm (about 30 to 25.6 inches) at three months — but almost every patient also had liposuction, so not all of that difference comes from the ribs. Other series report reductions of 6 to 10 cm (roughly 2.5 to 4 inches). The honest answer is not to promise a number: the consultation measures how much of your waist is fat, how much is rib, and how much is muscle and posture.

Healthy people at a stable weight, close to their ideal weight, whose waist is limited mainly by the shape of the rib cage rather than by fat. Published series exclude patients with chronic respiratory disease, previous rib surgery, a high body mass index or spinal problems, and recent evidence suggests checking bone health too, because the whole result depends on a fracture healing well. If most of your waist is fat, the answer starts with liposuction.

It carries risks that other contouring operations do not, and you should know them before deciding. A 2025 systematic review reported pneumothorax in 1.9% of cases, skin burns in 6.7%, asymmetry in 2.3% and severe postoperative pain in 9.2%. In 2026 the Aesthetic Surgery Journal published six deaths associated with the puncture technique in Peru, Ecuador and Mexico, reviewed by scientific committees that included the Colombian Society of Plastic Surgery. This is not an operation to choose from a photo: it is decided after a full consultation, in an accredited surgical facility, with a surgeon who knows what to do if something goes wrong.

Removal — resection of the floating ribs — was the first way to narrow the waist through the skeleton, and it is the most aggressive: it needs a larger incision, takes out bone that does not come back, and has described risks such as hemothorax and pneumothorax. Remodeling keeps the rib and changes its angle with a controlled partial fracture, through punctures or a small incision, sometimes with plate fixation. Remodeling techniques also differ in approach and in evidence, and that is explained in the consultation.

The pain is rib pain — similar to a bad bruise — and it is the most uncomfortable part of the first weeks, especially when coughing, laughing or changing position. The compression garment or corset is worn continuously for several weeks, because it holds the ribs in their new position while they heal, and neglecting it is one of the causes of asymmetry. Desk work usually resumes within days; exertion and exercise that load the trunk wait considerably longer. The result is assessed from month three, which is when published series measure it.

Yes, and that is the most common plan. The waist is the sum of three things — fat, ribs and muscle — and remodeling only addresses one. That is why it is almost always planned together with high-definition lipo 360 or male lipo 360, and sometimes with fat transfer to the buttocks. Combining changes the operating time, the anaesthesia and the recovery, and that is defined in the surgical plan, with safety first.

There is no single price, because different techniques share the same name and it is almost never done alone. The price depends on the technique, how many ribs are treated, whether it is combined with liposuction or another operation, the operating time, and what the quote includes: surgeon fees, accredited surgical facility, anaesthesiology, supplies, garment or corset and follow-up visits. Dr. Ramos provides a personalised quote after the consultation, which can be virtual.
